Our wellness initiatives address the various social factors that impact one’s life, understanding that health is more than just our medical condition.

We work with people to address a number of needs, including, but not limited to:
• Chronic Health Conditions
• Dental
• Food and Diet
• Health Education/Literacy
• Housing
• Mental Health
• Need a Doctor
• Social Isolation
• Tobacco and other drugs
• Unemployment
• Violence and/or Abuse
