Community Health Program
At the Louisville Urban League, we know that while there are many great services and programs in our community, they don’t always reach the people who need them most. That’s where It Starts With Me! comes in.
This community health program bridges the gap—connecting individuals and families to the support they need to live healthier, happier lives.
Launched in 2016, It Starts With Me! is powered by a team of dedicated Community Health Workers who meet people where they are. They provide one-on-one support to help improve health habits, overcome barriers, and reduce health disparities in our city.
Since its start, the program has:

Helped individuals move from the streets into safe housing

Supported families facing domestic violence

Connected clients to food, clothing, doctors, and dentists

Guided people toward stable employment

And so much more
This program is completely free to the community.

Community Health Workers
How it Works
“It starts with me!” has three basic phases that define the program’s approach. After the 90-day program, participants will complete a final survey and be discharged from the program.

We want to help you and your family live your best life by connecting to resources in the following areas:
- Health Education
- Feeling Disconnected from Family or Community
- Race Related Stress
- Housing Stability
- Physical Activity
- Tobacco, Alcohol and other Substances
- Anxiety and Depression
- Unemployment
- Exposure to Violence or Abuse
- Diet and Nutrition
- Financial Resource Strain
- Chronic Health Conditions (Diabetes, COPD, Heart Disease, etc.)
